29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  The employer failed to develop lockout/tagout procedures for equipment, exposing employees to caught in and amputation hazards in the following instances:  1) In the basement, the Trumatic 260A Turret Punch Press, which has hazards from the stored electrical, hydraulic and air pressure of the machine has no lockout tagout procedures.  2) On the second floor, the Fedal VNC 3016 CNC Vertical Mill had the interlock safety device on the door overridden, allowing the employee engaged in servicing and cleaning to turn the lathe on with the doors open.
