29 CFR 1926.451(c)(2)(v): Forklifts used to support scaffold platforms did not have the entire platform attached to the forks. (a) Jobsite - On or about January 6, 2015 two employees were elevated 7.7 feet above the ground while standing on sheets of plywood that were placed on the forks of a forklift. The plywood was not secured to prevent displacement from forks.

(a) Roof - On or about January 6, 2015 employees were working on a low-slope roof without guardrail system or personal fall arrest system or safety net system. Employees were exposed to falling 14 feet to a lower level.
