29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. On or about January 9, 2015, at 4510 S. Lee Street, Buford, Georgia: Three employees performing masonry work were exposed to fall hazards of up to approximately 21-feet when they worked from the work platform of a tubular welded frame scaffold that was not attached to the building they were working on.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: On or about January 9, 2015, at 4510 S. Lee Street, Buford, Georgia: Twelve employees performing masonry work were exposed to roll-over and struck-by injury when an employee and supervisor operated a JLG Skytrak Model 6036 powered industrial truck without performing an inspection prior to the use of the industrial truck.
