29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Employees engaged in material handing activities in the salvage yard were exposed to struck-by hazards in that the employer had not assessed the workplace to determine the need for high visibility vests when employees were traveling from the office and shop to the heavy equipment/material movers while recycling activities were occurring.
