29 CFR 1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:  (a) Facility, 12261 US Route 422, Kittanning, PA:  On or about 01/12/15 employees who wear prescription lenses were not wearing eye protection that incorporated the prescription in its design nor were they wearing eye protection that could be worn over their prescription glasses.  The employees are operating or working around equipment that generates wood pieces and particles and the appropriate eye protection is not being utilized.

(b) Facility, 12261 US Route 422, Kittanning, PA:  On or about 01/12/15 an employee was adjusting the guide wheels on the band saw mill and the equipment had not been locked or tagged out to isolate the energy.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) Facility, 12261 US Route 422, Kittanning, PA:  On or about 01/09/15 there was no point of operation guarding on the splitting saw to protect employees from the 52 inch rotating blade.  An employee who was assisting the operator with the sawing operations suffered an amputation to their left index finger when contact was made with the rotating blade.  There were no guards or other barriers in place to prevent contact with the danger zone during sawing operations.

29 CFR 1910.265(e)(5)(ii)(a): The top(s) and opening(s) in end and side frame(s) of edger(s) were not adequately guarded:    (a)  Facility, 12261 US Route 422, Kittanning, PA:  On or about 01/23/15 the top on edger #1 was not adequately guarded in that the guard does not cover enough of the saw blade to protect employees from the rotating blade.  The operator of edger #2 is positioned approximately 1 foot away from edger #1 exposing the employee to potential contact with the rotating blade.
