29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    On or about January 13, 2015 during the installation of second floor sheeting material at a job site located at 144 Vine St, Westlake Ohio the employer (The Coral Company) did not ensure that the sub-contractors (Noble Construction) employees whom were working directly below the sheeting installation were protected from head injury by providing and requiring the use of protective helmets.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     On or about January 13, 2015, at the job site located at 144 Vine St. Westlake, Ohio 44145 during the installation of the second floor subflooring materials on a newly constructed residential home, the employer (The Coral Company) did not ensure the sub-contractors (Noble Construction) employees were protected from potential eye injuries while using an air powered nail gun to attach the subflooring materials to the joists.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501     On or about January 13, 2015 during the installation of second floor sheeting material at the job site located at 144 Vine St. Westlake, Ohio on a residential new construction home the employer (The Coral Company) did not ensure the sub-contractors (Noble Construction) employees were protected from a fall hazard of up to approximately 12 feet by utilizing a fall protection system/device.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail.     On or about January 13, 2015, at the job site located at 144 Vine St. Westlake Ohio 44145, the employer (Coral Construction), did not ensure that the sub-contractors (Noble Construction) employees were not exposed to a hazard by which employees were utilizing a stairway with no handrail to access the second floor during the framing and sheeting of a newly constructed residential home.
