On or about January 13, 2015, employees were exposed to the hazard of falling approximately 60 feet while working from a suspended scaffold that was attached to a 7 story apartment building. The two outriggers that were being used to support the scaffold each had one wire rope tieback which was attached at an angle.

The lifelines that the exposed employees were utilizing as part of their personal fall arrest system shared the same anchorage point with each other along with the two wire ropes that were being used to secure the tiebacks.
