29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Jobsite:  Employees working on the ground clearing debris from roofing activities being conducted on the roof of a three level house were not protected from falling debris and objects by protective helmets exposing them to struck-by hazards.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)  Jobsite:  Employees performing roofing operations on a three level residential building were not trained by a competent person as required by this section exposing employees to fall hazards of greater than 30 feet to the frozen ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    Jobsite: Employees were using an aluminum extension ladder to access the roof approximately 30 feet above.  The ladder was not long enough to extend more than 3 feet above the landing, was not secured at the top, nor were any devices for aiding the employees to mount or dismount provided exposing employees to falling approximately 30 feet to the frozen ground below.
