29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Front side (cut-bench), new townhome construction located at 105 Brindle Lane, Alpharetta, GA 30009: Employee using a Makita (Model 5007F) 7-1/4-inch Circular was exposed to struck-by eye injuries while operating this tool without protective eyewear. (b) Front side (pump Jack scaffold), new townhome construction located at 105 Brindle Lane, Alpharetta, GA 30009: Employee using a Hitachi (Model NT65M2S) 2-1/2-inch Finish Nail Gun and a Enginuity (Model BFN16) Finish Nail Gun was exposed to struck-by eye injuries while operating these tools without protective eyewear.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Lower roof, new townhome construction located at 105 Brindle Lane, Alpharetta, GA 30009: Employee was exposed to falls of approximately 21-feet, 2-inches to the lower level while installing wood trim around a window from a 6/12 pitched roof with no fall protection system in place.
