29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area: (a) Signs by Tomorrow, at 1955 Arapahoe Street Unit C, Denver, CO: On and preceding 01/13/15, the employer did not ensure that employees were provided effective information and training as specified in 29 CFR 1910.1200(h) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area. Employees worked with chemicals during sign making processes. The employer had not provided hazard communication training. Employees worked with, but not limited to, the following chemicals: 1. "Rapid Remover" Adhesive remover; 2. Paint thinner

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d): (a) Signs by Tomorrow, at 1955 Arapahoe Street, Denver, CO: On and preceding 01/13/15, The employer did not verify that the required workplace hazard assessment had been performed through a written certification. Employees were exposed to chemicals such as but not limited to spray paints and solvents. The employer had not certified that a personal protective equipment hazard assessment had been performed. Employees were exposed to the following, but not limited to: 1. Exposure to fumes while laser cutting acrylic sheeting; and 2. Exposure to vapors from paints and solvents.
