29 CFR 1910.24(h): "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23.    a.     In the 550 Area for Storage and Maintenance, the stairway platform to the upper storage area did not have a mid rail to protect employees when unloading storage materials thereby exposing employees to a ten foot fall hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.    a.        In the machine shop, the electrical disconnect to the brake press had a unused openings in the bottom of the disconnect thereby exposing employees to a contact hazard.    b.        In the machine shop, there was an unused opening in the bottom of the Shear disconnect thereby exposing employees to a contact hazard.    c.        In the machine shop, the main disconnect which was not marked had unused openings in the energized disconnect in the bottom and side of the disconnect.    d.        In the machine shop, the disconnect for the furnace had unused openings where circuit breakers were suppose to be in place thereby  exposing employees to a contact hazard.    e.        In the machine shop, the disconnect for the welder had unused openings to energized electrical thereby exposing employees  to a contact hazard.    f.        In the 550 area for storage and maintenance the Siemans disconnect had unused  openings in the side of the energized disconnect thereby exposing employees to a contact hazard.    g.        In the 550 area for storage and maintenance, the Square D disconnect had an unused opening in the bottom of the disconnect thereby exposing employees to a contact hazard.

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.    a. In the machine shop on the main electrical wall, the employer installed a piece of steel across the front of the energized electrical circuit box and did provide a dead front to keep employees from coming into contact with live circuits thereby exposing employees to a contact hazard.
