All-American Hose, LLC, 2424 West 23rd Street, Erie, PA 16506, maintenance shop area, on or about January 14, 2015, and times prior thereto: Two oxygen cylinders were stored within two feet of three acetylene fuel cylinders.

All-American Hose, LLC, 2424 West 23rd Street, Erie, PA 16506, maintenance shop area, on or about January 14, 2015, and times prior thereto: Compressed gas cylinder containing gases including, but not limited to, oxygen, acetylene, and argon / carbon dioxide mix, were free standing and were not secured by any device to prevent the cylinder from falling, rolling, or getting knocked over.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv): A.) All-American Hose, LLC, 2424 West 23rd Street, Erie, PA 16506, on or about January 14, 2015, and times prior thereto: A flexible extension cord was running from an inside receptacle, along a wall surface, through a closed garage doorway and outside to a delivery truck equipped with a diesel block heater. The cord was exposed to physical damage due to pinch points between the overhead door / door frame, and created a trip / fall hazard. The cord did not comply with 29 CFR 1910.305(g)(1)(iv) in that the flexible cord was running through a doorway, and was used as a substitute for fixed wiring of a structure.
