29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: On or about January 14, 2015, at the above addressed job site, employees performing asphalt patching operation were not provided with gloves, exposing them to come in contact with asphalt material (Sakrete All Weather Blacktop Patch) that can cause skin irritation and dermatitis.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (1) On or about January 14, 2015, at the above addressed job site, employees using a STIHL TS-420 Cutoff Machine were not provided with eye protection, exposing them to flying projectiles and silica dust that can result in corneal abrasion and laceration to the eye. (2) On or about January 14, 2015, at the above addressed job site, employees using VP 1550 Low Vib Asphalt/Soil Plate Compacter were not provided with eye protection, exposing them to flying projectiles that can result in corneal abrasion and laceration to the eye.
