29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  Employees performing production tasks from elevated work platforms were exposed to fall hazards of up to 15 feet to the concrete floor below in that the ladder openings were not protected from falls for the following locations:  a) Production Line #2 work platform in the southeast corner of the room had a 7 foot fall hazard. b) Production Line #2 work platform in the northwest corner of the room had a 13 foot fall hazard. c) Production Line #2 work platform in the southwest corner of the room had a fall hazard of 11 feet. d) Production Line #3 bottom work platform for the extruder had a fall hazard of 10 feet. e) Production Line #3 top work platform for the extruder had a fall hazard of 15 feet. f) Production Line #1 bagger work platform had a fall hazard of 12 feet. g) Production Line #1 extruder work platform had a fall hazard of 13 feet.
