(a) Location: 65 Leland Road, Chestnut Hill MA, 02467 On or about 1/14/2015, a competent person did not perform a frequent and regular inspections of the job-site to identify safety and health hazards such as (Stairwell missing guardrails, damaged extension cords and improperly spliced power cord to a circular saw).
