29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:   a. At the back of the building, employees were exposed to a fall hazard, in that, a three frame scaffold was used during masonry wall construction without having guardrails at three sections on the second working frame.   b. At the front of the building, employees were exposed to a fall hazard, in that, a bracket platform was erected on the third frame of a three frame scaffold without railings at the end of the platform.   c. At the perimeter of the building, employees were exposed to a fall hazard, in that, a three frame scaffold was erected during cement block wall construction where the cross bracing was used as a top rail on the second working frame without having a  midrail installed. 29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  a. North side of the building, employees were exposed to a fall hazard, in that, cross braces were not installed on three scaffold sections at the top frame where material was placed during construction of the north wall.
