The employees have not been given a medical evaluation to determine if the employee is able to use a Gerson half face respirator before being required to use it, as determined on or about 01/12/2015. The respirator was hung on a nail in the paint mix room without protection from damage, dust and contamination, as determined on or about 01/12/2015.        ABATEMENT CERTIFICATION REQUIRED
