On or about January 14, 2015 and at times prior employees were exposed to fall hazards of greater than 4 feet from an unguarded floor used for storage above the tool cage area with an open side having only a single loose single chain in place of a standard railing.

On or about January 14, 2015 and at times prior to employees were exposed to trip and fall hazards from non-uniform stair tread riser and landings exhibiting different heights and widths near top of stairs for the maintenance shop storage area.
