29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  On or about January 14, 2015 and at times prior employees were exposed to fall hazards of greater than 4 feet from an unguarded floor used for storage above the tool cage area with an open side having only a single loose single chain in place of a standard railing.

29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs.  On or about January 14, 2015 and at times prior to employees were exposed to trip and fall hazards from non-uniform  stair tread riser and landings exhibiting different heights and widths near top of stairs for the maintenance shop storage area.

29 CFR 1910.305(g)(1)(iv)(E): Flexible cords and cables were concealed behind building walls, ceilings, or floors   On or about January 14, 2015 and at times prior to employees were exposed to electrical shock hazards from electrical wiring being ran unprotected through a metal wall opening and under plywood flooring at the top of stair landing for  the maintenance shop storage area.
