29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) On or about 01/15/2015, at the above addressed work site, the employer did not initiate and maintain a safety program for employees in regard to fall protection.  b) On or about 01/28/2015, at the above addressed work site, the employer did not initiate and maintain a safety program in regard to fall protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system.  a)  On or about 01/15/2015, at the above addressed work site, employees working on a sloped roof did not have fall protection exposing them to a fall hazard from the roof peak of up to 20-feet.  b) On or about 01/28/2015, at the above addressed work site, employees working on a sloped roof did not have fall protection exposing them to a fall hazard from the roof peak of up to 20-feet.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and was not withdrawn from service until repaired.  On or about 01/15/2015, at the above addressed worksite, a defective extension ladder was being used exposing the employees to a fall hazard of up to 10-feet.
