29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6):    (a) Employees are not performing lockout/tagout when removing stuck dough on the Reading Bakery Systems Line 6 Pin Roller and Die Roller Cutters (SH201-040-2008-3924, serial number J103422-070), in the South Mixing Room, this exposes employees to entanglement and amputation hazards.    (b) Employees are not performing lockout/tagout when removing jammed product bags in the Cloud Packaging Equipment, Roberts Packaging Machine #2 (model number IMFS-1500, serial number 10114), this exposes employees to entanglement and amputation hazards.    (c) Employees are not performing lockout/tagout when removing jammed product bags in the Cloud Packaging Equipment, Roberts Packaging Machine #3 (model number IMFS-1500, serial number 3534), this exposes employees to entanglement and amputation hazards.    (d) Employees are not performing lockout/tagout when clearing product jams on the Heat & Control, Fastback Conveyor (model number CV-FB-200A15, serial number 1209), this exposes employees to struck-by and amputation hazards.    Baptistas's Bakery, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 315119453, citation number 1, item number 2 and was affirmed as a final order on June 10, 2011, with respect to a workplace located at 4625 W. Oakwood Park Drive, Franklin, WI 53132.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Employees were exposed to entanglement and amputation hazards from lack of guarding:    (a)    The ingoing nip point on the Reading Bakery Systems Line 9 Pin Roller and Die Roller Cutters (model number SH202-039-2012-6507, serial number J113034-090), in the North Mixing Room, was not properly guarded to prevent employees from accessing the danger area;    (b)    The ingoing nip point on the Reading Bakery Systems Line 6 Pin Roller and Die Roller Cutters (SH201-040-2008-3924, serial number J103422-070), in the South Mixing Room, was not properly guarded to prevent employees from accessing the danger area;    (c)    The bag discard shoot on the Cloud Packaging Equipment, Roberts Packaging Machine #2 (model number IMFS-1500, serial number 10114), did not have a guard to prevent employees from accessing the danger area;    (d)    The bag discard shoot on the Cloud Packaging Equipment, Roberts Packaging Machine #3 (model number IMFS-1500, serial number 3534) did not have a guard to prevent employees from accessing the danger area;    (d)    The slide gate on the Heat & Control, Fastback Conveyor (model number CV-FB-200A15, serial number 1209), did not have a guard to prevent employees form accessing the danger area.
