29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about January 15, 2015, at the above addressed site, an employee was exposed to a fall hazard of approximately 15 feet when performing electrical work from the bucket of a truck mounted aerial lift while not being protected from falling to that lower level.
