On or about 1-14-2015 an employee of Jayhawk Boxes, Inc. a subsidiary of The Lawrence Paper Company was injured while attempting to troubleshoot the ink dispenser on the 208 Hooper Swift Cardboard Box Printer/Slotter ( unk serial#). The employee's clothing was snagged by a rotating shaft as the employee reached across the shaft in order to check ink levels. The machine was not de-energized prior to performing this operation.
