29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On the 208 and 220 Printer Slotter Machines: The employer failed to ensure that authorized employees affixed Lockout or Tagout devices prior to performing maintenance and or servicing of equipment. On or about 1-14-2015 an employee of Jayhawk Boxes, Inc. a subsidiary of The Lawrence Paper Company was injured while attempting to troubleshoot the ink dispenser on the 208 Hooper Swift Cardboard Box Printer/Slotter ( unk serial#). The employee's clothing was snagged by a rotating shaft as the employee reached across the shaft in order to check ink levels. The machine was not de-energized prior to performing this operation.
