29 CFR 1910.133(a)(5):  The employer did not ensure that each affected employee uses equipment with filter lenses that have a shade number appropriate for the work being performed for protection from injurious light radiation.  a) On or about January 12, 2015, or prior thereto, the employer did not ensure that employees using an oxygen-acetylene torch utilized eye protection with a shade protection against radiant energy.  This torch was utilized during the transmission repair of a 2003 Chevy Silverado.  Abatement certification is required for this item.

29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, start up, or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative.  a) On or about January 12, 2015, in the workplace:  Energy control procedures have not been established by the employer for automotive repairs in the maintenance shop.  Hazardous energy release due to unexpected motion from gravity was not prevented prior to a transmission repair of a 2003 Chevy Silverado pickup.    Abatement certification AND documentation for this item are required.

The Hyster is utilized to lift automotive and heavy equipment parts during repairs in the maintenance garage.  Abatement certification is required for this item.

29 CFR 1910.244(a)(2)(vi)(a):  Each jack was not thoroughly inspected at times which depend upon the service conditions. Inspections did not occur at least once every 6 months for use that was constant or intermittent at one locality.  a) An orange hydraulic jack was not legibly marked and had not been inspected.  The jack was utilized during repairs to the John Deere backhoe in the maintenance garage.  a) A 6,000 pound capacity low profile service jack had not been inspected.  The jack was utilized during automotive and truck repairs in the maintenance garage.  c) A 2,000 pound capacity Central Hydraulic automatic transmission service jack had not been inspected.  The jack was utilized during automotive and truck repairs in the maintenance garage.  Abatement certification is required for this item.

29 CFR 1904.39(a):  Within eight (8) hours after the death of any employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor.  a) On the afternoon of January 12, 2015 a work related fatal accident occurred during a transmission repair on a 2003 Chevy Silverado.  The employer did not report this incident to OSHA within 8 hours.  OSHA learned of the incident 2 days later from another source.  NOTE: This item was abated during the inspection.
