Employees did not control all hazardous energy sources such as, but not limited to, electrical and hydraulic before performing activities such as, but not limited to, changing set-ups and cleaning machines. Employees were exposed to amputation hazards.

Ashland, OH 44805: On or about January 9, the employer failed to obtain manufacturers approval to use Caldwell 432 lift truck front end attachment with forklifts such as, but not limited to, Raymond stand-up forklift (Model EASI R30TT, Serial # ET-C-99-07969). The forklift with the attachment was used for maintenance activities such as, but not limited to, replacing a cylinder in the Mazak Ultra 650 CNC machining center. Employees were exposed to crushing, struck-by, caught in between, and tip over hazards.

Ashland, OH 44805: On or about January 15, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chucks of equipment such as, but not limited to, three Powermatic drill presses (Model # 1200, Serial # 4-5331) and one Rockwell drill press located on the light drills table in the East machine shop. Employees were exposed to struck-by and amputation hazards. B) Pentair, Inc., located at 1101 Meyers Parkway. Ashland, OH 44805: On or about January 15, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chuck of the American horizontal lathe machine located in the maintenance shop. Employees were exposed to struck-by and amputation hazards. C) Pentair, Inc., located at 1101 Meyers Parkway. Ashland, OH 44805: On or about January 2, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chuck of the Bridgeport vertical mill machine located in the maintenance shop.
