29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems: a. On or about January 15, 2015, employees were exposed to the hazard of falling approximately 32 feet while loading and unloading roofing material in the hoisting area, on flat residential roof, without being protected from falling. James Roofing Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 315498055, citation number 1, item number 1 and was affirmed as a final order on May 04, 2011, with respect to a workplace located at 2816 Countrybrook Drive, Palm Harbor, FL, 34684.

29 CFR 1926.502(k)(3): A copy of the fall protection plan with all approved changes shall be maintained at the job site: a. On or about January 15, 2015, employees were exposed to the hazard of falling approximately 32 feet while tearing off the old roof and installing tar on a flat residential roof. The employees were using the safety monitoring system but did not have a written fall protection plan at the jobsite.
