29 CFR 1926.1053(a)(19): Where the total length of a climb on a fixed ladder equaled or exceeded 24 feet (7.3 m), the fixed ladder was not equipped with either:  (i) a ladder safety device;  (ii) self-retracting lifelines, and rest platforms at intervals not to exceed 150 feet (45.7 m); or (iii) A cage or well, and multiple ladder sections, each ladder section not to exceed 50 feet (15.2 m) in length.    a) Building roof, gravity water tank:       Employees were exposed to a fall of approximately 35 feet while accessing a fixed ladder to the top of a water tank.
