29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219:    In the northeast section of the production shop, employees operated an Extrema double-sided planer that had an unguarded belt and two pulleys at the bottom of the machine, exposing employees to being caught in the in-running nip points.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the maximum protection possible for the operation being cut.    In the southeast section of the production area, the De Walt radial arm saw did not have a self-adjusting blade guard, exposing employees to hazards of amputations, lacerations, and/or broken bones.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a)  Near the south end of the production area, near the downdraft table, a gangway outlet box that was used to connect a sander to the power source was suspended from the rafters and was not mounted and used as designed by the manufacturer, exposing employees to electrical shock and fire hazards.    (b)  Near the center of the production area, an outlet box that was near the clamp machine was tied to the machine by the flexible cord, not mounted and not used as designed by the manufacturer, exposing employees to electrical shock and fire hazards.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    On the west side of the production area, access to the electrical panel boxes to disengage the power in an emergency was blocked by a table which was bolted to the floor, exposing employees to electrical shock and fire hazards.
