29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: At the job site 2900 N. Rotary Terrace, Pittsburg, Kansas, employees had operated the maintenance shop bench grinder without the tongue guard properly adjusted. Employees were exposed to stuck by hazards from grinding debris and wheel brake up.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the job site 2900 N. Rotary Terrace, Pittsburg, Kansas, employees in the printing shop were exposed to amputation hazards in that machine guarding was not provided at the following pieces of equipment. a. Press 226, Bindery Area top cover was missing. Employees exposed to caught in hazards. b. Press 226, Gear side print area not covered in two areas. Employees exposed to caught in hazards. c. Press 222 South End, 4th print unit not guarded. Employees exposed to caught in hazards. d. Press 245, Stevens 1000, missing nip point guards. Employees exposed to caught in hazards. e. Press 227, Stevens 1000, missing nip point guards. Employees exposed to caught in hazards. f. Collator 413 had missing top cover guards for the cutter and slitter area. Employees exposed to caught in hazards.
