29 CFR 1910.23(b)(1): Every wall opening from which there is a drop of more than 4 feet was not guarded; a) On or about 1/08/15 at building #2719 on the second floor, employees unloading sand and gravel from a bag on a forklift pallet elevated through the opening of the wall were exposed to a 17 ft. fall hazard, in that, fall protection was not provided. One employee fell out the wall opening.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a. On or about 01/08/2015 at building #2719 on the second floor, an employee fell 17 feet to the parking lot area and was admitted to the hospital on 01/08/2015. The employer did not report the incident until 01/14/2015.
