29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: In the 300 Ton Area, the Rutland Milling and Drilling Machine, which is used to drill out plastic boxes, was not equipped with a point of operation guard, exposing employees to a caught in and struck by hazard.

29 CFR 1910.1200(e)(1)(i): The written hazard communication program did not include a list of the hazardous chemicals known to be present, using an identity that was referenced on the appropriate material safety data sheet: In the establishment, as a part of the written HAZCOM program, the employer had not developed a listing of chemicals present in the operations area, exposing employees to the potential hazards of printing catalyst, which is flammable or naphta, which can cause central nervous system damage.
