29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d): (a) Moffat Mine Service Inc, at 315 Tucker Street, Craig, CO: On and preceding 1/14/15, Moffat Mine Service Inc did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d). Employees conducted operations which included exposure to hazardous chemicals. Hazardous operations included, but are not limited to the following: 1. Exposure to metal fumes and argon when MIG welding mild carbon steel; 2. Exposure to metal fumes when plasma cutting mild carbon steel; 3. Exposure to oxygen and acetylene when torch cutting mild carbon steel; and 4. Exposure to paints and lubricants when maintaining shop equipment.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard in the workplace: (a) Moffat Mine Service Inc, at 315 Tucker Street, Craig, CO: On and preceding 1/14/15, Moffat Mine Service Inc did not identify and evaluate the respiratory hazard in the workplace. One employee operated a Shop Sabre Sidekick CNC plasma cutting machine to cut mild carbon steel. This process generated metal fumes. The employer did not evaluate employee exposure to metal fumes including copper fume, iron oxide fume, and manganese fume. This condition potentially exposed employees to a respiratory hazard. (b) Moffat Mine Service Inc, at 315 Tucker Street, Craig, CO: On and preceding 1/14/15, Moffat Mine Service Inc did not identify and evaluate the respiratory hazard in the workplace. One employee MIG welded mild carbon steel. This process generated metal fumes. The employer did not evaluate employee exposure to metal fumes including copper fume, iron oxide fume, and manganese fume. This condition potentially exposed employees to a respiratory hazard.
