29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by paragraph (l): On or about January 15, 2015, and times prior thereto, the employer did not ensure that each operator of the electric pallet jack had successfully completed the required training prior to operation. Employees were exposed to the hazard of being struck by falling load and/or pallet jack.
