29 CFR 1910.147(c)(4)(ii): Energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about 01/16/2015, the employer did not ensure that lock-out/tag-out procedures specific to the Hydraulic Shop Press - 55 Ton (Part No. 60361, Serial No. 0111AH32987) were developed and implemented; procedure to include a specific statement of the intended use of the procedure, specific procedural steps for shutting down, isolating, blocking, and securing the machine, specific procedural steps for the placement, removal, and transfer of lock-out/tag-out devices and the responsibility for such devices, and specific procedural steps for testing the machine to determine and verify the effectiveness of such devices and other energy control measures.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Instance 1: On or about 01/16/2015, in the shop, the employer did not ensure that the Jet Drill Press (Model No. JDP-17MF, Serial No. 2043070) employees use to drill holes in various pieces of equipment was equipped with a guard to protect the operator and other workers in the area from the hazards created by the point of operation (drill bit) and flying debris.
