29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips: (a) On or about January 14, 2015 and times prior thereto, the Grizzly G4003 lathe did not have a guard to protect the operator. This exposed the employee to a potential caught-in hazard from the rotating part of the chuck.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replace: (a) Maintenance shop: On or about January 14, 2014 and times prior thereto, the Lincoln arc welder Runger 8 model welding lead had a cut to the conductor 2 feet from the electrode holder. This exposed employees to electric shock hazard from coming in contact with the conductor.
