29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: (a) North hoist: On or about January 13, 2015 and times prior thereto, the blank was missing on the electrical panel exposing the conductor. This exposed employees to electrical shock hazard from incidental contact with 240 vac system.

29 CFR 1917.42(i)(2): Screw pin shackles, used aloft in house fall or other gear, did not have their pins moused or otherwise effectively secured. (a) North Hoist: On or about January 13, 2015 and times prior thereto, the screw pin shackle was not secured to prevent the pin from backing out. This exposed employees to potential struck-by hazard if the shackle disengaged during lifting operations.
