29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. At this workplace, employees conducted oil and gas operations from passageways above tank equipment, and manhole covers tethered permanently by a looped cable to the aisle floor created tripping hazards.
