On or about January 14, 2015 at 2108 Avondale Mill Road, Macon, GA: Room 315- Filter Room B - Employees working around with the rotary air lock were exposed to an electrical shock and burns when:   a. the cords entering the service disconnect #4 was not fitted with a bushing that would protect it from abrasion.  b. the cords from motor to the rotary air lock was not fitted with a bushing that would protect them from abrasion.
