(a) Porch roof, new residential construction located at 355 Lynne Circle, Alpharetta, GA 30009: Employee installing 1/2-inch plywood sheathing was exposed to eye injuries while operating an Hitachi (Model NR83A2S1) 3-1/4-inch Framing Nail Gun at 120 PSI with no protective eyewear.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) East side, new residential construction located at 355 Lynne Circle, Alpharetta, GA 30009: Employee was exposed to severe laceration and amputations while operating a DeWalt (Model DWE575) 7-1/4-inch Circular Saw without the lower guard in place. A wood shim was used to keep the lower guard from returning below the base plate after a cut was made through the plywood which exposed the operator to a moving blade which was not guarded.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Porch roof, new residential construction located at 355 Lynne Circle, Alpharetta, GA 30009: Employee was exposed to falls of approximately 11 feet while installing plywood sheathing on a 6/12 pitched roof without a fall protection system in place.
