29 CFR 1910.23(d)(1)(iii): On stairways less than 44 inches wide having both sides open, one stair railing on each side.     On or about January 15, 2015 and prior to that time located in quadrant D adjacent to the catwalk entranceway employees accessed both sides of an elevated walkway platform crossover by utilizing a 24 wide set of temporary stairs having four risers. The temporary stairs on both sides of the crossover were not equipped with standard stair railings on either side of stairway system exposing employees to a fall potential in excess of 32" from the stairway.

29 CFR 1910.24(i): "Vertical clearance." Vertical clearance above any stair tread to an overhead obstruction shall be at least 7 feet measured from the leading edge of the tread.    On or about January 15, 2015 and prior to that time located in quadrant D there is a single exit rout from the catwalk entranceway floor area to the 5th floor utilized by employees rigging in the truss work or operating follow spots.  The single exit route egresses contains a fixed stairway and hatchway floor opening as a part of the means of employee exit wherein the ceiling height at the top of the fixed stairs has vertical clearance of 38" above the stairway stair tread and overhead obstructions of a structural steel beam and metal decking .

Any projection from the ceiling must not reach a point less than six feet eight inches (2.0 m) from the floor.    b) On or about January 15, 2015 and prior to that time located in quadrant D there is a single exit rout from the catwalk entranceway floor area to the 5th floor utilized by employees rigging in the truss work or operating follow spots.  The single exit route egresses contains a fixed stairway and hatchway floor opening as a part of the means of employee exit discharge wherein the ceiling height at the top of the stairway is 38" above the stairway stair tread.

On or about January 15, 2015 and prior to that time located in quadrant D there is a single exit route from the catwalk entranceway floor area to the 5th floor utilized by employees rigging in the truss work or operating follow spots. The single exit route egresses contains an entrance doorway from the catwalk floor area to a hatchway floor opening as a part of the means of employee exit wherein there is a 28" high structural beam which completely obstructs the means of egress exit route not allowing for a minimum 28" wide exit access discharge.

On or about January 15, 2015 and prior to that time employees were traversing point to point on elevated structural truss work and accessing between the elevated truss work by climbing point to point on cross braces between the truss work sections without personal protective fall protection equipment being utilized exposing employees to fall potentials in excess of 75 feet.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a) On or about January 15, 2015 and prior to that time, employees performing rigging work accessing on structural steel truss work and cross bracing between the truss work were utilizing lanyards wrapped around the structural anchorage point of the truss or cross brace which were not being maintained in a reliable condition, in that, the lanyards were exposed to the edge of the angled structural edge without being protected from abrasion exposing employees to a fall potential in excess of 75 feet.   As referenced by the American National Standard Institute, (ANSI) Z359.1 - 2007 Safety Requirements for Personal Fall Arrest Systems, Subsystems and Components, Section 7.2 Equipment Rigging and Use. The PFAS equipment shall be rigged to prevent its contact with hazardous objects in the workplace, which could damage the equipment or prevent its proper function.      b) On or about January 15, 2015 and prior to that time, employees performing rigging work accessing on structural steel truss work and cross bracing between the truss work were utilizing lanyards wrapped around the structural anchorage point of the truss or cross brace which were not being maintained in a reliable condition, in that, the lanyard being utilized was wrapped around a structural member and connected back to itself exposing the connector to accidental disengagement hazard between the lanyard and connector keeper exposing employees to a fall potential in excess of 75 feet.   As referenced by the American National Standard Institute, (ANSI) Z359.1 - 2007 Safety Requirements for Personal Fall Arrest Systems, Subsystems and Components, Section 7.2.5 Lanyards and lifelines shall not be wrapped around structural members and connected back to themselves unless a suitable anchorage connection means in integral to them.    c) On or about January 15, 2015 and prior to that time, employees performing rigging work accessing on structural steel truss work and cross bracing between the truss work were utilizing lanyards wrapped around the structural anchorage point of the truss or cross brace which were not being maintained in a reliable condition, in that, the lanyard being used was attached around the structural member with simultaneous frontal and dorsal positioned lanyard connectors attachments  to the harness exposing employees to fall arrest loading harness fall out potentials in excess of 75 feet.  As referenced by the American National Standard Institute, (ANSI) Z359.1 - 2007 Safety Requirements for Personal Fall Arrest Systems, Subsystems and Components, Section 3.2.2.4 The fall arrest attachment shall be located at the back (dorsal) position.
