29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: On and before January 15, 2015, the employer failed to ensure Ladle Dump steel alloy chains were inspected daily prior to use on overhead crane #291. The Warren Steel Holdings, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.179(j)(20(iii), which was contained in OSHA inspection number 974586, citation number 01, item number 003 and was affirmed as a final order on June 23, 2014, with respect to a workplace located at 4000 Mahoning Ave. Warren, Ohio 44483.
