29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): A.) Roll Forming Corporation - On or about 1/15/15, the company's energy control procedures were deficient in that they did not include specific information including, but not limited to, the types and magnitudes of energy, specific steps for the lockout of machinery, specific sequential steps to bring the machines to a zero energy state, specific steps/techniques for isolating stored energy, specific steps to verify de-energization of the equipment , specific steps for the release from lockout, and specific steps for isolating or eliminating energy sources. Employees performed servicing and maintenance on equipment that included, but was not limited to, shear presses.
