29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  (a) 3170 Carlisle Pike New Oxford, PA - An employee was utilizing a JLG 450 AJ- aerial lift to finish attaching the metal siding with screws.  The employee was not protected/restrained inside the basket by the use of a fall harness and attached lanyard, thereby exposing to fall hazard of 16 to 18 feet, on or about January 16, 2015.  No abatement certification or documentation required.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:   (a)  3170 Carlisle Pike New Oxford, PA  - Employees were observed performing roofing activity on a flat roof with a 1 in 12 pitch without the benefit of a fall protection system, thereby exposing employees to a fall of 19 feet to the ground below, on or about January 16, 2014.    No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  3170 Carlisle Pike New Oxford, PA - The employer did not provide training to employee(s) which would have allowed employee(s) to recognize the hazards of falling and the procedures to minimize those hazards, thereby exposing employees to possible injuries, on or about January 16, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
