29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, and flying chips: 1) On or about January 6, 2015, near the storage area of the machine shop, a Wilton belt sander (#5511365) did not have a have a side guard installed to protect the employee(s) from rotating parts and nip/pinch points. 2) On or about January 6, 2015, in the work shop, a Carlton drill press (#IA399) was not guarded at the point of operation to protect the employee(s) from rotating drill bit and flying metal chips. 2) On or about January 6, 2015, in the shop area, the employer did not ensure that all lathe machines were guarded to protect the employee(s) from rotating chuck and flying metal chips.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about January 16, 2015, next to the belt sander in the machine shop, a Baldor pedestal abrasive grinder (#G0-231-60) did not have a tongue guard in place to protect the employee(s) from flying debris in the event of wheel shattering during grinding operations.
