29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about 01/19/15, in the sodium bay area, an employee was not wearing a personal fall arrest system (full body harness) while opening a latch on top of an ISO sodium tank at approximately 8' to 10' above the ground. b) On or about 01/19/15, in the sodium bay area, two employees were on top of an ISO sodium tank at approximately 8' to 10' above the ground without wearing a full body harness while rescuing an employee that had fallen inside the ISO sodium tank. ABATEMENT DOCUMENTATION REQUIRED
