29 CFR 1910.132(f)(1)(i): The employer did not provide training to each employee who is required by this section to use PPE on knowing when PPE is necessary:    a) On or about 01/19/15, in the sodium bay area, employer did not provide specific training requiring the use of personal fall arrest systems (full body harnesses) for employees standing on top of the ISO tank at approximately 8' to 10' above the ground while performing work and/or performing rescue tasks during emergencies.    ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.146(k)(1)(iii)(A): Rescue team selected did not have the capability to reach the victim(s) within the time frame that was appropriate for the permit space hazard(s) identified;     a)  On or about 1/21/15, in Bay 3, an employee was injured after falling inside of the permit required confined space sodium tank and the employer did not have an adequate confined space rescue plan in place because the employer relied on Niagara Falls Fire Department for emergency rescue and they declined to rescue the injured employee.       ABATEMENT DOCUMENTATION REQUIRED
