29 CFR 1910.23(c)(1):  Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides of platform:     a/ Luxor/Linden NJ Site: Warehouse Suite C. Linen Sheets Area:  An employee was observed standing on the open-side of a stack of cardboard boxes filled with product, while attempting to retrieve a box of product that was located approx. 8 ft from the floor.     Means were not provided to prevent employee(s) from standing on unapproved platforms to store and/or retrieve product.      Observed on/or about 01/20/2015.

Street Side: Exit door leading to the street facing another facility was locked with a sliding bolt preventing immediate escape in the event of a fire or other emergency. 29 CFR 1910.36(g)(2): The exit access was not at least 28 inches wide (71.1 cm) wide at all points:  a) Luxor/Linden NJ Site: Warehouse Suite C. Linen Sheets and Pillow Areas: Employee work stations were not provided with a minimum of 28 inches of walking space where only 12 to 16 inches of space were estimated to be maintained. The work station aisles were partially blocked by empty cardboard boxes and partially filled boxes of product.   Observed on/or about 01/20/2015.

Shipping/Receiving Dock:  Electrical panel boxes (voltage 480 Volts/3 phase) were not maintained free of storage material, e.g., boxes of fluorescent light bulbs, U-Line Products and a small portable fan.     A minimum 30 inches of space was not maintained in front of the electrical panels.    Observed on/or about 01/20/2015.
