(A) In front of the building an employee elevated in the basket of the HR2 Model 400S Aerial Lift was not wearing a body belt or full body harness with a lanyard attached to the boom or basket to protect him from being discharged from the basket at 17 feet above the ground level, on or about January 20, 2015.  (B) Inside of the building an employee was observed operating HR2 Rough Terrain Aerial Lift without a body belt or full body harness with a lanyard attached to the boom or basket to protect him from being discharged from the basket during travel, on or about January 20, 2015.
