OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by and crushing hazard: Location: 240 West Sixth Street, Boys and Girls Club. On January 14, 2015, an Employee was fatally injured while building concrete forms directly below a suspended load of 26 sheets of drywall supported by a boom lift truck, when the load fell.
