29 CFR 1910.159(c)(10): Sprinklers were not spaced to provide a maximum protection area per sprinkler, a minimum of interference to the discharge pattern by building or structural members or building contents. Second floor conference room and hallway, 164 Fredette Street, Gardner, Massachusetts; Sprinkler heads for the automatic fire suppression system in the second floor conference room and hallway are blocked by ceiling tiles. The ceiling tiles prevent the flow of water from the sprinkler heads over the designed area, exposing employees to burns and other hazards from fires.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Production floor, Garlock Printing, 164 Fradette Street, Gardner, Massachusetts; Points of operation on machines were not sufficiently guarded to prevent employees from coming into contact with pinch points and in running nip points. 1. Press 1 - The take up roll is exposed at the point of operation where an employee could come into contact with an in-running nip point. 2. Press 3 - Printing decks were not sufficiently guarded. Deck six was the location of the amputation. 3. Converting 2 - Stapler guard was not sufficient to prevent access to in-running nip points.
