29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  a)  On the coil forming production floor, on or about January 8, 2015, guillotine cutter safety guard on Machine #12, Marlon 2001, was not designed to completely encase cutter, allowing forming machine operators to reach around the safety guard and exposing fingers to point of operation.     b) On the coil forming production floor, on or January 8, 2015, the forming machine heater/drum on Machine #12, was not guarded to prevent employee from leaning against or touching a heated energy source exposing the employee to potential burns.  c) On the coil forming production floor, or about January 8, 2015, the forming machine heater/drum on Machine #14, was not guarded to prevent employee from leaning against or touching a heated energy source exposing the employee to potential burns.  Abatement Certification AND Documentation is required for this item.
