29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    The employer did not ensure that energy control procedures clearly and specifically outlined the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance.  This violation was discovered on or about January 20, 2015 and at times prior thereto, where the employer's energy control procedures did not:    Clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to ensure compliance;    Contain a specific statement of the intended use of the procedure;    Contain specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;    Contain specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and    Contain specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and to ensure that machine or equipment components were operationally intact:     The employer did not ensure that before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees inspected the work area to ensure that nonessential items had been removed and to ensure that machine or equipment components were operationally intact.  This violation occurred on or about January 12, 2015, where at least one employee did not replace the machine guards to the Pocket Boring machine before restoring power to the machine.

This violation occurred on or about January 20, 2015 and at times prior thereto; in the Mini Mill/Frames department, a metal electrical receptacle box designed to be installed inside of a wall was used on the end of an electrical cord powering Pocket Boring machines.
